Introduction

For tax year 2015 Keystone has the ability to print 1095-C forms and e-file 1094-C/1095-C information for both regular and self-insured employers. This page will provide assistance with entering the required ACA information into Keystone. It is your responsibility to gather this information and to assure that it is accurate. You should consult your accountant before filing ACA information.

IMPORTANT: Please make sure that your Keystone has been updated to version 2.9.5 or higher.

Form Printing

1095-C

Use the "Print 1095-C Forms" option on the Payroll-Year End menu to print 1095-C forms. Keystone is configured to print 1095-C forms on pre-printed inkjet/laser single-sheet forms.

1094-C

At this time Keystone does not print 1094-C forms. You should prepare this form manually.


E-File

Before you can E-File

In Keystone please verify that employee social security numbers and names are accurate. Your entire batch of 1095-C forms will be completely rejected if social security numbers and names cannot be validated by the IRS!

There are several steps that must be taken which allow a company to electronically file 1094-C/1095-C information. Here is an outline of these steps along with IRS links to help you:

  1. IRS Registration https://la1.www4.irs.gov/e-services/Registration/index.htm
  2. Confirm IRS Registration (after confirmation code received from IRS) https://la1.www4.irs.gov/e-services/Registration/index.htm
  3. IRS E-Services Registration https://www.irs.gov/Tax-Professionals/e-services---Online-Tools-for-Tax-Professionals
  • Select Application
  • Select ACA Applpication for TCC
  • at least 2 Responsible Officials
  • at least 2 Contacts
  • Role: Issuer
  • Transmission Method: AFA for ACA Internet Transmitter

Preparing the XML File

IMPORTANT: Do not run the 1095-C E-File Export until notified otherwise on this page! The IRS test site has not yet been activated and therefore we are unable to validate the export file.

Use the "1095-C E-File Export" option on the Payroll-Year End menu to prepare the XML file. NOTE: You will need to enter your TCC (Transmitter Control Code) to prepare the XML file.

Uploading the XML File

The 1094-C/1095-C XML File must be uploaded directly to the IRS using the website upload method.
